This system-wide model integrates clinical excellence, technology, and teamwork to simplify care for patients while empowering providers to practice at the top of their license.Key Responsibilities* Provide comprehensive Internal Medicine care for adult patients, managing both acute and chronic conditions.* Collaborate with a multidisciplinary care team including advanced practitioners, behavioral health, nursing, and population health specialists to deliver coordinated, value-based care.* Utilize advanced technology and analytics to enhance patient engagement, access, and outcomes.* Participate in system innovation, quality improvement, and care redesign initiatives.* Enjoy a flexible, physician-centered schedule that supports work-life balance.
